
The Le'Chris Community Support team provides support and
services to adults with mental health and substance abuse
diagnoses who are in need of an intense level of service.
Consumers entering this service must have a mental health or
substance abuse diagnosis and must have had difficulties
with some of the following:

Homelessness
-
Multiple psychiatric hospitalizations

Medication difficulties
-
Non-compliance with psychiatric follow-up
-
Educational, vocational, or legal issues
-
Recent suicidal thoughts
-
Threats to others
-
Dual
diagnoses of mental illness and substance abuse
The goal for the consumer is to achieve and maintain
stability in their mental health treatment and/or meet their
sobriety and recovery goals.
Additionally, the goal is to assist the consumer with having
decreased crisis episodes and increased community tenures.
Services will be provided at various locations including
home, shelters, schools, streets, etc. and can be provided
for four (4) hours or more per week.
The role of the Community Support Team is to:

Assist the consumer in learning to manage the symptoms
of their illness; medication monitoring; medication
compliance; self-medication and self-management of
symptoms; and, education to the consumer about their
diagnosis

Assist the consumer in identifying their aspirations,
goals, strengths and needs

Provide individual therapy
-
Teach
coping skills for dealing with depression, anxiety, and
anger
-
De-escalate crisis and teach the consumer coping skills
should a crisis arise in the future
-
Assist in the development of independent living skills
-
Case
management functions of assessing needs, linking to
needed services and monitoring the provision of these
services
-
Family education
-
Peer
support building
-
Self-advocacy training

Each consumer is seen by the members of the Community
Support Team. The leader of the team is a licensed
counselor, and all other team members have experience,
education, and training in working with adults with mental
illness or substance abuse problems.
